There are so many amazing pharmacists to celebrate this month! Today, we highlight a member of LLUSP's Class of 2008, Dr. Remmington Junior, Board Certified Ambulatory Care Pharmacist (BCACP), a Board Certified Advanced Diabetes Management Pharmacist (BC-ADM), and a Certified Diabetes Care and Education Specialist (CDCES)!
"After pharmacy school, I completed a PGY1 in Ambulatory Care, worked at CVS/Pharmacy as a Pharmacist-In-Charge, and transitioned into my current role as a privileged and credentialed mid-level practitioner embedded in an outpatient Internal Medicine primary care clinic, co-managing chronic diseases in older adults. My clinical expertise, particularly in anticoagulation and cardio-renal-metabolic care, is a cornerstone of my practice. I spearheaded four pharmacist-led clinics and successfully implemented remote patient monitoring services to increase access to care. I work closely with clinical leadership and population health to influence prescribing and enhance quality and patient safety. As the Anticoagulation Stewardship Pharmacist, I lead the development of anticoagulation policies, SOPs, and report tools to adhere to NPSG 03.05.01. I also co-chair the Population Health Working Group (PHWG) Clinical Practice Guidelines (CPG) Team for Diabetes, Hyperlipidemia, and Obesity, where I evaluate clinical registry quality program data to develop and deliver educational programs and promote evidence-based medication use across the organization.
What I enjoy about pharmacy is clinical programming and building personal relationships with patients, which I find deeply rewarding and essential to understanding how culture, faith, and personal goals improve health outcomes."
