Randy P. McDonough, BS, PharmD, MS. BCGP, BCPS, FAPhA
Professor, Pharmacy Management and Innovation
Loma Linda University School of Pharmacy
Co-Owner and Director of Clinical Services
Towncrest Pharmacy

Dr. McDonough left his academic position as full professor (clinical) at the University of Iowa College of Pharmacy in 2006 to prove that clinical services could be successfully implemented and paid for in community pharmacy.  Since that time, he has been a co-owner of several community pharmacy practices, co-developed software for clinical documentation, started consulting and educational companies, and successfully challenged a major payer to pay for pharmacy services.   Currently he is a Professor for Pharmacy Management and Innovation for Loma Linda University School of Pharmacy, Director for Practice Transformation for the national program Flip the Pharmacy, while actively maintaining ownership and practice within his community pharmacies.

Course Description:

The survivability of community-based pharmacy is being challenged in the health care system.  The traditional "Retail" pharmacy model is being threatened by market consolidations, vertical integration, market disruptors.  Also, community pharmacy practices that are focused on product distribution are no longer meeting the needs of a changing health care system that is embracing value-based reimbursement models.  Reimbursement for product distribution continues to be reduced adding to the threats to survivability. Therefore, pharmacists and technicians practicing in community-based pharmacy practices need to transform their pharmacies to create the capacity to provide patient care.  This requires practice changes that improve practice efficiencies, integrate clinical services within workflow, and optimize pharmacy personnel with the goal of freeing up the pharmacists to provide clinical services that optimize patients drug therapy.  Ultimately, this "transformed community-based pharmacy practice" is contributing to the health and wellbeing of patients and recognized and paid by key stakeholders.  This program will provide the participants with the knowledge and insight on the key tenets for successful community-based pharmacy practice transformation, the impact it is having on patient care and health care costs, and how community-based pharmacy practice transformation is occurring on a national level.

Course Objectives:

Upon successful completion of this knowledge-based course pharmacists and pharmacy technicians should be able to:

  1. List and discuss the five tenets of community-based pharmacy practice.
  2. Describe how these five tenets create the capacity within the practice for pharmacists to provide clinical services.
  3. Describe how these community-based pharmacy practices that have transformed are finding new opportunities working with other stakeholders and payers.
